This is a diary created by a psychology teacher with the intention of getting our students to reflect on their mental wellbeing each day, and to act as an opportunity for form tutors to identify students who need support. This diary has 5 check-in slips for each day of the week per page. I have the booklets out on desks according to their seating plan for students to complete as a ‘do now’ acitivity as soon they enter the form room. This is a part of their daily form time routine. By simply ticking the box from 5-0 that describes how they are feeling, this allows me to identify which students I need to speak to and identify the appropriate support. There is enough slips in this booklet for the entire academic year and it can simply be reprinted and photocopied each year. At the end of the year, students can take their diaries home or rip off the first page with their name, then recycle the rest of the booklet as there is no personal information in the rest of the booklet.

Simply download and laminate. I put these out on desks before tutor period. As students enter, I have them remove jackets and bags, take out equipment on the mats and complete their ‘do now’ which is the wellbeing check-in diary (uploaded in shop). I simply go around the room and check all equipment is on desks. If not, students are sent to the school equipment shop. If your school does not have an equipment shop, you could lend any missing equipment by holding onto student ID or travelcard until equipment is returned at the end of the day.
